CDD portal is an interactive but double blind health portal. As an USER of this portal you can store, retrive, modify and update your health data in a portable format. Along with this you may put your queries to the community. One user may comment on the queries of other, along with VISITOR may also put comment to the user. However no one can access other's identity.
To use this portal one has to register in this portal.
To register in this portal as USER or VISITOR (to comment on the queries of user) please make ready SOFT COPY of your identity information and a photo (in jpg/ tiff/ png format).
To register, go to 'Registration' link and fill out the necessary fields to fill your basic details.
During registration you may opt to display your data in Public or Confidential. ONCE YOU SELECT THIS, THIS IS NOT EDITABLE FURTHER.
After registration, portal ID will be assigned and health data can be uploaded which you may retrieve, modify and update at any time in future.
Please do not share your portal ID. Based on your mobile number and email ID portal ID will be assigned. SO PLEASE DECIDE ABOUT THIS. PLEASE NOTE THAT OTHER BASIC DETAILS ARE ALSO NOT EDITABLE.
IF YOU WANT TO CHANGE EMAIL ID/ MOBILE NUMBER/ HOME ADDRESS, PLEASE CONTACT TO ADMIN FROM THE EXISTING REGISTERED EMAIL AND THIS CANNOT BE ENTERTAINED NOT MORE THAN ONCE IN A YEAR AND NOT WITHIN THREE MONTHS OF REGISTRATION.
If you want to serve the community with the data of any deceased person for whom you are the legal heir you can also register for that in the Posthumous section and upload data. This is for the advancement of science and community. The identity of the Posthumous will not be displayed. Please note that if any controversy arises with the posthumous data upload or display, portal ADMIN will not be responsible.
If anyone use abusive language or content in this portal his/her registration will be cancelled and portal Admin's discretion in this matter is to be considered as final and non argumentive in all respect.
